A FRACTURE crack on a coal train bogie went undetected during earlier maintenance efforts, the Australian Transport Safety Bureau has found.
According to the Bureau, on 12 December 2017, a fractured bogie was identified on a Pacific National during a roll-by inspection at the Kooragang Coal Terminal in New South Wales.
This wagon operated loaded between Tahmoor Colliery and Kooragang.
The ATSB found a fatigue crack went undetected during preventative maintenance prior to the structural failure of the bogie frame.
Pacific National made changes to the maintenance standards to increase the area of the bogie frame.
“Asset managers should ensure that inspection techniques effectively monitor and report on asset condition,” the Bureau stated.
“Risk controls should also be continuously assessed to control risk to an acceptable level through the life cycle of the asset.”
